Fingerprint Background Waiver Required

The Fingerprint Background Waiver in the Personal History Record form must be signed and dated before the fingerprints are completed. If the waiver form is dated after the fingerprint card, the fingerprint cards will be rejected.

Note for Nevada residents

Nevada residents are now required to obtain prints through an acceptable LiveScan vendor. Additional information about the fingerprinting process can be obtained from the Nevada Resident Fingerprinting document below.
